Every report submitted in good faith is read by a human. You do not need to use legal terminology or identify a protected category to report. If anyone is in immediate danger, contact 911 or the appropriate local emergency service before using this form.

No retaliation

Chez prohibits retaliation against anyone who files a good-faith report, participates in a review, or asserts a right under our nondiscrimination and equal-access policies. If you are concerned about retaliation, flag it below and a separate monitor will be opened alongside your report.
